The enthusiasm, however, has outpaced the evidence. The overwhelming majority of human studies are observational: they document that depressed patients tend to harbour a microbial profile distinct from that of healthy controls, but they cannot adjudicate whether the altered flora precipitates the low mood or merely registers its downstream effects. Depression alters appetite, sleep, and physical activity—each a potent reshaper of the gut ecosystem—so the arrow of causation may well point in the opposite direction, or in both at once. Proponents invoke plausible mechanisms: the vagus nerve as a conduit, microbial metabolites that cross the blood–brain barrier, the modulation of inflammatory cytokines. Yet a mechanism, however elegant, is not a proof of effect. Randomised controlled trials of probiotic 'psychobiotics' have so far yielded effect sizes that are modest at best and frequently evaporate under rigorous blinding. The field's besetting sin is to mistake the abundance of correlational data for the convergence of causal demonstration. Until interventional studies in humans consistently reproduce the dramatic reversals seen in mice, prudence counsels treating the gut–brain axis as a fertile hypothesis rather than an established therapeutic highway.
